The arrangement commits the FQHCs, community organizations and Fidelis treatments in a unique partnership to address social care needs by advancing social determinants of health (SDOH) interventions as managed care services, with the potential to alleviate food, housing, transportation and health insecurities. socio-economic conditions experienced by patients served by Yuvo Health Members of the FQHC.

The arrangement, which is a two-year partnership beginning as a Tier 1, upside-down only arrangement and evolving as a Tier 2 mitigated risk arrangement, incentivizes improved population health outcomes and reduced risk. Avoidable Hospital Utilization through a Total Cost of Care for the General Population Model of Medicaid. The arrangement will also focus on 13 clinical quality measures, including avoidable hospitalizations, readmissions and avoidable emergency room visits, as well as NCQA HEDIS measures.
